Community Health Needs Assessment

St. Luke Community Healthcare is committed to the health and well-being of Mission Valley citizens and, as the largest provider of healthcare in the valley, we are responsible for proactively monitoring the health of the communities we serve. It takes a village to create a healthy community – a few minutes of your time can have a big impact. The Community Health Needs Assessment (CHNA) – which is conducted every three years – utilizes a broad range of questions to develop a clear vision of the needs of local residents and families. Our community is made healthier and stronger when all perspectives are heard. This survey takes about 10 minutes to complete. We appreciate your voice and thank you for helping shape the future of community health. 

In appreciation of the community’s feedback, St. Luke is giving away five $50 cash cards. If you would like to enter the drawing, please enter your email at the end of the survey. Your responses will not be attached to your email, and all your information will remain confidential.
